i personally love it. i also wish you could do it to people's pets, and every npc that sings that damn red diamonds song. and that whiny chick when you land at the wayshrine at belkarth. and to peoples mounts. that would be awesome.
would also be nice to be able to attack someone from your horse.

the BoW's cutaways are tame as hell compared to some other games. even skyrims kill animations where brutal. It is part of the game though not mandatory, my friend did all of the DBH quests without using the BoW.
How could your friend do all those quests without using the blade? The first thing you have to do is use it to advance in the questline.

The Blade of Woe isn't that bad, in terms of gore. At least it serves as a reminder that you are killing someone. I think it's reasonable that you should be made aware of the consequences of your actions (done for personal gain in terms of cash, drops and XP). Maybe that's the problem... some people don't want to think that they are committing murder...
I know it's just a game, I know it's just make-believe, but I found that the most worrying aspect of DB was having to kill a random innocent right at the start. Killing an innocent person just to become an assassin. Antisocial behaviour, impaired empathy and remorse, and bold, disinhibited, egotistical traits - that's the definition of a psychopath!
It took me a while to get over that psychological hurdle (I know, sounds silly), but curiously all the people complaining about the gore associate with the Blade of Woe haven't mentioned having any qualms over killing an innocent to get it...
You know what's even more disturbing than the Dark Brotherhood?

All I would say is:
It would be a bigger problem IMO if the cult of assassins was not disturbing. Trying to make a group of people not seem that way that should is a really bad thing.
Turn off the kill animations, it's in settings.
And last: it's a mature game.
