Indeed, the core issue is that for some reason Garyn stops attacking Balreth. If that happens it will bug out. You need either kill Balreth before Garyn becomes inactive, or use that trick above.
When Balreth dies, Garyn must be in combat and actively fighting Balreth or it fails!
Grouping (and having a group's DPS) may help here.

Can confirm that it worked
It did show contain balreth's spirit after i killed him.
1. I was playing a dragon knight
2. A combination of fiery chains and the talisman to keep the aggro on me.
3. Aggro and deal enough damage to kill him
4. Nothing odd happened. He died like he would normally and i heard the npc say his usual thing.... after that the npc vanished and there was nothing to do.

He must be in a combat "pose" at the very least. I did this solo and here's what I did:
- Fight him normally
- When NPC bugs out and starts following balreth back and forth in a non-combat pose -- kill yourself and log out.
- When you come back he should be fighting balreth again
- if he bugs out in a combat pose and doesn't move, you can kill balreth and balreth's soul should spawn.
- You will have to waypoint out after talking to Mavos and run back up the mountain afterwards if the exit portal disappears.
