CONFIRMED SOLUTION:
It is confirmed the cause is the NPC getting out of combat. To get the NPC back into combat, get Balreth low, die, port out of the portal at the entrance, port back in and the NPC should be tanking Balreth or attacking him. At this point you need to kill him rather quickly spamming anything you have to kill him before the NPC stops attacking again.
I've attempted to not cast Magicka spells in hopes he would stay in combat and not restore Magicka, I've attempted not to use the Talisman, and this seems to be the only solution that works.

After doing this quest 5 times, this has about been the only solution that's worked for me, and I did it solo. Stand in the spot the little light that comes off the boss goes and spam E like there's no tomorrow.one person stays close to the NOC Mage. As soon as BALRETH is dead, the person next to the NPC spams E. Everyone should get credit.
