1. Are awards per account or toon (can I get multiple if I place two Sorcs in top rankings or a Sorc and a Templar)?
2. Awards are awarded only when vMA is the weekly trial, correct?
3. What is the other vMA leaderboard for - is that the monthly leaders or something?

Per character: every character who is in top 100 of their class will get the weekly reward.
2. Awards are awarded only when vMA is the weekly trial, correct?
Yes.
3. What is the other vMA leaderboard for - is that the monthly leaders or something?
It's for all the time ever (but I think it was reset in the past)
