1. Are awards per account or toon (can I get multiple if I place two Sorcs in top rankings or a Sorc and a Templar)?
2. Awards are awarded only when vMA is the weekly trial, correct?

Per character: every character who is in top 100 of their class will get the weekly reward.

Yes.

It's for all the time ever (but I think it was reset in the past)
