Very rare, had two experiences I remember. Note some saying group suck before leaving does not count here.PC-NA
I am very doubtful that this happens in any large amount, to be honest. The vast, vast majority of PUGs I run anymore hardly say anything besides "TYFG" at the end of the dungeon to "Hello" at the beginning, even with groups that cannot do the content. I would be very interested to see what behavior people encounter so often, especially the claims of over 50%

In my line of work i know how to handle those type of people.
But in games i do have some tricks to deal with them (so they leave the game), without breaking rules of any kind.

Me: Apple?

Me: Potato?
Toxic player:$&%&%/$&$/$$&
Me: Tomato?
Toxic player:%&/%&$/$($###"
Me: Banana?
Toxic player leaves the group/game
Trick is to not give them a satisfaction, wich funnels them. Making them fell akward cools them down.
