Sound issues are fixed in 3.2.9. Stop complaining.The second Omnivox alarm after you go over the bridge for the quest “Lost in the Gloam” is no longer a silent alarm.

In PvE too, but it happens only when you have a "zerg" of NPCs, the best example is the big pack of Adds just before the Mantikora in Sanctum Ophidia, almost everybody lose all sound at some point and it will automatically come back close to the end of the fight.
