What kind of latency do you have? I am wondering if it's not just timing but a disparity in the amount of time people have to click the synergy.

My biggest problem with synergies is that using them usually messes up my rotation, they don't flow into combat very well as opossed to weaving in your own abilites while light/heavy attacking.

I was thinking about this.
Good post; highlights one of the main ... general issues with higher level/vet ESO combat. It requires a high degree of manual dexterity and serious dancing button-pushing.
If "it" (whatever "it" might be) doesn't mesh well with this high-speed tango, then "it" will go by the wayside ... for many players.
Some of us more mellow types will still use the non-optimal things, or everyone will when there is no choice, but the way that the game is structured to require speed typing is creating some problems here.

So maybe it's the button that's part of the issue? What if it was 6 instead of x or button next to swap bars?

when I tank, I keep the boss as still as possible, a templar drops nova, no synergy prompt pops up.
well at least I can see nova, unlike some other AOEs with no synergies...

Oh this makes too much sense.
So I miss it, 'cause now it's displaying for someone else. Then they miss it, and so on till it runs out.
I think that might be what's happening.
