The headpieces will have a 100% drop rate from bosses in Update 12 (Matt Frior confirmed this in yesterday's video). Coupled with in-group trading, getting the piece that you want in the trait that you want should be pretty easy now. As someone who primarily plays endgame PvE and only a little bit of PvP, dueling is actually pretty exciting. It allows for casual PvP with friends, and I can see it as a gateway drug to more PvP. Imagine two friends who PvE together and don't PvP. As for the shoulders, they're dividing the chests up so that each chest has a specific set of dungeons associated with it. So you can home in on a shoulder and only open the chest that can drop that shoulder.
I'm not too worried about it, frankly.
