My fps has increased from 40-45 to 60 in cities, but at the same time CPU usage has increased from 50-60% to stable 90-95%. I am not sure if it's healthy to play that way, so I am thinking of turning it off again.
Yes FPS is fully unlocked with this setting. I have a GSync monitor (144Hz) and my fps goes beyond that sometimes as well (GSync on, VSync off). I've limited in game fps to 150 via Votan's Advanced Settings addon. I was having issues in VAS+2 yesterday with LAs and Synergies not firing correctly, but I will chalk that up to patch day blues.
Weird thing is I actually saw my frames go above 200 a few times. I have GSync enabled, and monitor refresh rate at 180. Never seen it go above 180 in other games (unless I OC my monitor to 200hz). VSync in game had no effect on this, it would go over whether or not I enabled it. I typically disable VSync when I am running GSync. Was seeing 200+ both on my NVidia overlay and the ingame /FPS.

For some reason every time I try to toggle it on and restart my game the setting is toggled off.When I try to turn it on in user settings documents it's not there for me to turn on. I tried repairing the game, but would it be better if I did a clean install of the game?

Your problem is, that your Documents folder is protected against changes by eso64.exe.
Check your anti virus settings. Is there an exception rule for ESO and/or the Elder Scrolls folder?
My multithread setting keeps resetting to off every time I launch game, is this normal?
Edit: nevermind, looks like my settings file did not get updated with new settings. Copied new stuff manually from PTS file like a nerd I am.

No, it is as simple as adding one line to setting file:
SET RENDER_THREAD "1"
However, the actual problem is AVAST antivirus blocking write access to setting file. To fix it, add C:\Users\<user>\Documents\Elder Scrolls Online\live folder to exception list.
