I generally pick a forum name based on my current most long lasting RP character. I don't try to swap my identity from platform to platform, just if I am registering a new account in a new place I might up it to my newest real main, the one most of my friends know.
My RP main in GW2 since pre-beta has been Donari Mal di Bette (reworked from a Birthright campaign character named Donari Malo di Bette which is 20 characters long and GW2 limits to 19, who in turn was reworked from a WoW blood elf named Fashenal Swiftcutter). So on GW2 forums and here, as well as in Discord and in Roll20 game chats, I'm Donari and have been since 2012 when he became my main.
Because the love of goats and goat-related things is infectious.

My first online nick for irc & quake was demon. Coz quake was all about the demons! But it was so heavily used in gaming that I changed it to demonik.... then dem0nik & finally dem0n1k. I had to make a uplay account recently & every permutation of demonik i could think of was taken.

Its an added letter version of the the main city in the b@$tard heavy metal dark fantasy anime "meta-ricana" been using that gamer tag since early mmo days just kinda carries over to everything i do online sadly most people lost all sense of pronunciation when trying to say it on voice chat so i get called metta alot.

My old tag used to be Vid Gamer. That was pretty generic and unfitting for naming characters in MMO's so I just reversed it to Remag Div. I thought it sounded pretty fantasy-ish. lol
My first console RPG, Phantasy Star Online: Episode I & II (PSO), on Xbox in 2002.
I made a FOmar (male human, Force class) that was dressed in all-black robes. It was like past midnight when I finally got Xbox Live working with our router/modem and internet back then.
I must have been 8 years old at the time. I was born in 1994, and I started PSO on Xbox sometime in 2002. (I'm 24 now, FYI!)
So, I made a mage archetype character, everyone was asleep and it was very dark outside. So I named myself: Darkmage.
It was probably my first-ever all-nighter. Hundreds more later followed throughout the years. Haha.
There was a dark-type spell technique in PSO called Megid that had a chance to One-shot/One-Hit-Knock-Out enemies and players alike, and many players would always spam it in PvP lobbies. I remember joining a PvP lobby and a bunch of players were shooting and whacking & wailing on a modder/hacker player named "1337<>Ninja" who was wearing near-invincible modded gear (gear created or altered through modified game code/3rd-party programs like GameShark).
I came in as Darkmage and one-shotted him with the Megid dark-type spell. (I got super-lucky, basically.)
After some conversation that I can't exactly remember, he took a liking to me and invited me to his clan, which was "1337." (it means LEET, short for "elite"). This was around the time "L33T-SP34K" and internet-slang became a thing (when "owned you" became "pwned you" etc.) Those dam millennials. Anyways, I was 8+ years old at the time and joined a rag-tag team of modders/dupers/hackers/game-network-manipulators, etc. Lmao. I then became 1337<>MAGE. In PSO we had hacked/modified over-powered weapons/armor to one-shot all the enemy bosses and farm them over and over and over to level up quicker (the max lvl was like 200, IIRC, and it took eons to reach it -- legitimately or otherwise).
I then carried the Darkmage name over to Phantasy Star Universe on Xbox 360 in 2006 and my Darkelf Destro-magic character in Elder Scrolls Oblivion in 2006, and Final Fantasy XI, World of Warcraft, Skyrim, etc. And now ESO.
If it's an RPG with a magic/sorcerer-based archetype, you can guarantee Darkmage will be there!

Friend at work, her last name was Soriano. I put an A at the end to make it more feminine and it stuck. Played through Skyrim and first closed beta with her. Second closed beta test I was in it was taken.

When early access came I was up three hours early, 1 am my time, to make sure I could grab the name as my main! Went through broken character creation with her, discovered I liked her look, wanted to be a tank (but knew nothing about the classes and races) and luckily picked Imperial Dragonknight.
Originally thought of because I love judeo-christian mythology and have a huge interest in angels. To those who may not know, a Seraph(Seraphim plural) is a very high ranking angel. For some reason I got thinking of praying mantises and so i thought Praying Seraph sounded good. Same amount of syllables as praying mantis and "praying" fit with Seraph in theme. Been using it for 4-5 years now, ever since I retired my old online alias i used since childhood(Zeratul868 cause Starcraft was awesome)
Ive used my account name (minus the numbers if possible) for venues that required a handle since around age 16 after I installed a CB radio in my car. We used to play a CB radio game called "Skunk", which was basically a group of people with CB equipped vehicles playing hide-and-seek in a designated square mile radius of town. One at a time over the radio, the seekers would ask questions to the hider for clues of their hiding spot. First seeker to find the hider got to be the hiding "skunk" in the next round. Winning had the perks of bragging rights and saving gas money, plus some down time to make out with your significant other. I chose the handle because my car was an old Ford Mustang, skunk was best played at night, and I hoped to be an unexpected winner.
