I use a lightning staff and I have had every single one of those things happen to me. I just didn't know it was related to ESO and not my mouse going out

Yeah noticed this as well.
Now add to this that in an on-going trial fight or during prebuffing people are dropping synergy skills and the synergy hides other interaction possibilities (ready checks, ressing ...)
