I remember trying to get Foul trophies in Hew's Bane and it took me three weeks (attempting twice a day) because they were never up or just gave out the same green one over and over.
I really hope they can drop more foul spots for us (I'm sure there are plenty of icky/dire oceanic areas) or up the spawn timer.

No it isn't. Two holes by the docks, one by the bridge next to a river, two in Karnwasten and a few unconfirmed next to geysers. All take ages to respawn, and get fished out very quickly by the hordes of angry fishermen

Yes. That is my experience on live. I know only of one confirmed near the Alinor geyser (where a dead crab is in the water).
Also I found only one lake spot, the one near the delve, which is even harder to complete for achievement.
Do you know of more lake spots?

All of these holes DO spawn as normal, however they are over-camped and generally fished empty within 5 minutes of spawning. Foul is obviously the bottleneck but it's no worse than for example river spots in The Rift. Most zones have a type of water that is rare and hard to find, for Summerset that happens to be foul.

I did the smart thing and finished the achievement and got my full Pyandonean motif while everyone else was doing the quests. If you are just starting now, you'll find worms have become a near extinct species on Tamriel, nothing to do about that but pay the price or farm it yourself really. Or wait until the rush has died off a little.

Maybe there are more, but I haven't discovered them. Even after having visited the whole map with the Hissmir beverage. But who knows, maybe I just missed them, which would be so easy considering they are never up! This utter nonsense is not even a grind anymore, since most of the time we're not farming the holes, but just rushing from Point 1 to point 2 (blessed be this one which has two holes) to point three and back to point 1, praying Z'en and Kynareth and Hircine for the holes to have respawned, which, of course, they have not. So most of the time, we do absolutely nothing.
There are tons of places where it would make sense to have foul water. Corgrad. Any geyser. The Rellenthil sinkhole. The entire course of the river coming from Cloudrest, presumably poisoned by Z'Maja's pestilence. But no, there are only four, two of them actually in a place where it makes no sense to have foul water.
