Why can't I finish Maelstrom

I've 2 toons stuck at last level. I've got the levels down to under 15 minutes to get boss round. Khajit Stamina nightblade with VO, TFS, TBS, NMG, Hundings, Spriggans, Hulking, any monster etc. this guy rarely gets out of crystal phase, and has had awful luck whenever I do. Like ghost spawn right on top of Solkyn, or…
