Block Shield always up on a player?

What set or ability cause the new blocking shield graphic to always be displayed on a player when mounted or on foot.
It should not be possible to block while mounted as that dismounts you.
The player in question it never goes away.
Some form of glitch?

✭✭It's a bug that's been present ever since they implemented it on player characters. It seems to happen more frequently if you die while blocking.
This was from last November:
