No, that happens to me by simply loading into Deshaan or Reapers March.

✭✭✭✭That's actually happened to me before too except I wasn't killed and about 30 seconds later I dropped right back down to where I was. I'm assuming that was how they happened to "fix" a particular bug. You probably just took a couple extra seconds to realize you were dead and when it happened you were in the limbo between zones.
