You soposed to get this achivement if you kill last boss without curing his debuf, but eaven it you do it you don't grt the achivement. I tryed this achivment several times and still no result you don't get if eaven if you do everything corectly and none of your teemmates die for the whole dungeon.
To provide more detail: During the part of the quest where you need to defend the hall, you kill the waves of goblins and spiders, Hrogar says there's the leader, you defeat him, and Hrogar asks where his sons are. At this point, you'd expect the quest marker to change, but it doesn't. Hrogar is still marked as a companion, and if you leave the building, you are notified that you Failed that part of the quest - the quest marker points into the hall again. Sigh... logged in today, 5 minutes in cyrodiil, click on a gate of a keep, end up halfway down in the floor, get kicked few seconds later by the server "excessive spamming" error message. Few minutes in, super FPS lag starts. Go to PvE, quest notification messages stop working. Ignore all the issues, try to kill some mobs, who start to warp around.. ignore that.. get hit for 2800+1300 damage by some mob.
Continued by pulling out the few hairs I had left on my head.
If it wasn't for the PvP in this game, I would have been loooong gone. So many bugs and issues in this thing, djeez!

Does ZoS have any type of QA at all?

She can still be damaged by AoE, and summoned creatures can see her (or could). It's a nuisance, but not impossible. If you don't want to wait for a real fix and don't mind trying/dying several times, it's doable

Judging from the issues I see cropping up after each patch, I would say that the simple answer is "no". They seem to have no QA at all, or at least none that works. Patches with as many issues as the two "patches from Oblivion" (1.1.2 and 1.2.3) simply must have bypassed QA entirely. I see no other possibility how bugs of this magnitude could not have been caught before release.
