Low GPU usage / Low FPS

Here is my rig info:
i7-3770k, 16Gb, geforce titan, win 7 64, latest drivers
I play at 1920x1200 preset ultra + 100% view distance.
Generally I get 45-50+fps outdoors.
In some cities like
Elden Root:
Avg: 36.718 - Min: 27 - Max: 39 (fraps benchmark)

All things (simple/old graphics + my rig) considered I should be getting at the very least 60 fps.
Perhaps it is because of windowed borderless mode (the game doesn't have a real fullscreen).. and a poor coding.
All other games run just fine.
