The Bone Colossus at the HM version of Fang Lair has a almost undodgable and supposedly melee heavy attack called "Clash of Bones." this stupid thing hits me while I should be out of range not to mention can't be dodged reliably. Either fix this or take the silly Mob out of hard mode
It has a long windup of 2.25s, so if you dodge when you see the telegraph, you will have dodged too early. Generally speaking, if you dodge more than 1.5s or so before an ability, you will be too early. The trick with Clash of Bones is to not dodge as soon as you see the telegraph, but to instead count one second in your head and then dodge.
Long heavy-attack windups aren't a new thing, though. Earthgore's heavy attack and the heavy attack of the bone colossus in in Falkreath behave in the exact same way, and it's something that tanks get used to pretty quickly, if they opt to dodge the damage instead of blocking and eating it.
Once you get used to the timing, you should be able to dodge it very reliably. The only times I die to it are times when I don't notice it (usually, it's when I'm so focused on tracking the ghost wall that I don't notice that a colossus has snuck up behind me) or when it overlaps with other mechanics (e.g., if I get staggered by the icy breath).
I'm aware of how it works. That doesn't mean it should work that way. With the amount of nonsense going on in the dungeon i couldn't have to count for it and even when I dodge and run it kills me sometimes which means it has a stupid range

The upside of the long windup is that you get plenty of warning. The downside of a long windup is that it's harder to time it to avoid dodging too early. While I dislike having to count (and I have died a number of times to mistimed dodged), I do appreciate there being more warning.
My biggest gripe with it is that it does an insane amount of damage. You can't block it. You can't shield it. If you're not the tank, the one and only way to survive it is to avoid it--i.e., dodge. It's poor design. In contrast, in Velidreth HM, if you can't get out of a statue bomb, you can at least block and/or shield to survive. Velidreth's does have a 1-shot that can only be avoided with dodge roll, but that happens during lull in combat, whereas having to watch for and time the bone colossus heavy attacks while also dealing with the ghost walls and skeletons makes for a much more chaotic fight.
