If they add them in and don't let them be killable (like Skyrim) then they break immersion. But if they do add them in as killable (whether players kill them or not) then suddenly that would put the game as either adult only content or just outright banned, and at a bare minimum it would have various different activist groups up in arms creating a lot of bad press.
So to just leave them out does not break immersion and does not cause marketing problems.
Additionally they add very little other than atmosphere so what's the point if you are going to possibly face all of those problems.
I hear children all the time in area chat usually making fun of me after I get ganked, but just prior to them also getting slaughtered.
If you list closely you can hear their parents in the background taking their playstation away for vulgar language.

Seriously though, it is at times somewhat grating.
Especially when a quest plot is written for a "child" to rescue, and then gives us an adult looking person behaving like a kid instead... I mean, the "my mom still treats me like a child even though I am fully grown" thing works exactly that one time in Koglin village, but all the other "dults" incapable of walking two dozend paces withjout someone to hold their hand are somewhat annoying...
So, even though I really don't care for kids, I think they should add -some-. Not annoying ones, but... around. Update those in the quests, where it makes sense. Have some walk around "safezone" cities.
They can easily be unkillable like quest NPCs, or they can make it more involved and have them pretend to be attackable, but auto-dodge all attacks and if someone tries, immidiately scream fo rht eguard who will then murderdeathkill the would-be child-beater...
But having some -would- make the game feel more alive...
