The boss Razorback in Bloodmayne Cave in Cyrodiil simply doesn't spawn, period. This means it's impossible to get the Bloodmayne Cave Explorer achievment, and also the Cyrodiil Cave Delver acvhievment. Unless a GM can get online to force the boss to spawn, I guess the only choice is to give out these achievments manually, until it can be changed to only require the two bosses that DO spawn.
Here's a link to the most comprehensive thread I've seen about getting Cyrodiil Cave Delver acvhievment. I used it to get mine a few months ago. Maybe you can try PublicDungeonChampions add-on if you don't use it.

I doubt an add-on will make the boss any less bugged... Either ZOS need to make Razorback spawn, so that the achievment can be gotten, or, barring that (the original programmer most likely isn't with ZOS anymore, and the current employees may well not have any knowledge about the boss mechanics), or change the achievement so that the bugged boss is removed.
@ZOS_GaryA Close to a day in the same dungeon now... STILL no sight of the boss! BOTH other spawn locations have been checked and NONE of the other bosses are up either, so this is NOT and issue with another boss blocking the spawn! So can we get an acknowledgement that this boss is non-existent now, and that those who need the Cyrodiil Cave Delver achievment will have to get it added manually by a GM until it is fixed so that the non-existing boss is no longer listed?
We definitely understand the frustration with this. The bosses in public dungeons should spawn every few minutes. If a boss is missing when a player enters a room, it's possible that another players recently defeated the boss, and the dungeon just needs a little time to reset itself. Considering you've spent an extensive amount of time in the dungeon, we understand that this is unlikely to be the case.
However, some bosses will not spawn while there are other bosses or key events to trigger in the dungeon. We recommend that players explore dungeons thoroughly to make sure that they haven't missed a required step before the boss spawns.
I had the same feeling like you man. I was waiting in this cave like 4 days until I got last boss spawned. Someone said that bosses there spawn one by one, always only one at time (which is NOT true). They spawn all, however it seems, that every each of them has "random spawn time" or similar thing. Usually, I had Razorback spawned (like 90% of time). Worse issue was Sprigan boss (dont remember name), this one was up much lower (lets say 40% of time I was there). The last one, for me, was Ironlash, the plant. It spawned for me just once (didnt even see a corpse)...
Truth is, that this cave was hardest one found and done in whole Tamriel. Each other cave has, or seems to have, normal spawn timers. Maybe, it´ll be usefull, if @ZOS_GaryA will escalate this issue with this cave to devs, to check the spawn timers and behaviour. Its really not great to waste 4 days of paid game time in one cave to farm one single boss...
I got the achievment in the end, but bloody hell - you shouldn't have to spend more than a day (not continuously, though) in a Dungeon to get an achievment like this...

The Explorer title has to be earned.
I remember spending several hours waiting for bosses to spawn, going through the dungeons to kill any other boss in the way. Fun times. :')
Razorback won't spawn for me. This is the last boss I need to get the Cyrodiil Cave Delver achievement. Gone back over three times and he's still a no show, no matter how long I wait or how many other bosses/mobs I kill in Bloodmayne cave ...

Don't forget that there is a guest campaign that you can try Bloodmayne Cave on ... if your home campaign is not spawning bosses at an acceptable rate ...
I am sorry to necro this thread, but this is utter BS.
Razorback doesn't spawn period! I am full hour inside Bloodmayne Cave, there is nobody else beside me, I killed 3x both other bosses (Acanthia and Ironslash), so 6x overall, and yet Razorback doesn't spawn. Everything spawns inside cave (enemies and various resource nodes including chests) but not him. I don't have problem with other Cyrodiil caves in AD zone except this one. They are cleared pretty quickly except Bloodmayne Cave.
Three years later and bug still exists...

I earned the Explorer title over a year ago and I can confirm that a few Delves in southern Cyrodiil have strange boss spawn times. Although there should always be a boss up every 2-5 minutes (idk what the timer is anymore) it was not uncommon to wait 30+min for a single spawn in a few of the Delves. It would sometimes require the player to leave the Delve for bosses to spawn again too.
On top of this, it would either randomly select the boss to spawn or (based on my experience) heavily favor some bosses more than others. There were a few bosses I waited *hours* for, only to kill different bosses numerous times, eventually leave/return, and then find them spawned later in the day. Perhaps bosses are connected to the Day/Night cycle? Whatever it is, I 100% guarantee they are a few Cyrodiil Delve boss timers not working as intended..... unless it is meant to sometimes take hours (with other bosses being killed immediately) for a specific few bosses to spawn lol.
I am not sure how much it impacts the boss spawn rate, but I do the same approach to this delve and always end up with the boss kills before I leave in one session. Have done this with 7 toons that I have taken to Bloodmayne Cave.
Walk in, grab everything and burn it down.
Most times by the time I get back up from killing Acanthia, Razorback or Ironlash is already up.
Razorback spawns in the NW pocket of the first opening.
Ironlash spawns in the one to the right of it at due North.
Make sure to clear and kill any enemy in these pockets or they will not spawn there.
If you continue to clear those pockets and remain in the room you will get the bosses. I have never spent more than 30 minutes in Bloodmayne Cave to get the achievement.
In fact, I just got it. I was in here to check on Ironlash's spawn location because Razorback was up as soon as I came from killing the Nirn.

Daily reminder that Razorback doesn't exist. As AD player I regularly enter Bloodmayne Cave for AP boost and I do that with two different characters - nothing! It's always Acanthia or Ironlash.
Razorback = Yeti / Bigfoot / Loch Ness monster / El Chupacabra etc.

As of today, it is official that Razorback does exist. My time in the Bloodmayne Cave delve was about 1hr. I killed all that I saw at least once and then Acanthia spawned. I killed her and all that I saw from the beginning to the end once more and Ironlash spawned. I downed him and repeated the process again from beginning to end and Razorback spawned. Don't forget to LOOT EVERYTHING.
There's a trick related to bosses respawn in Cyrodiil's delves: the player needs to wait near a boss that he already killed in order to the other one to respawn. If there's more than 2 bosses on the delve, the player needs to alternate its waiting position between the bosses killed until the 3rd one respawns. So wait 15-20-25 minutes near a boss that you are not missing and after that check the place where the other one is supposed to respawn.
This worked for me, I hope it also works for you.
Very Important Note: As pointed before LOOT EVERYTHING once you kill a boss.
