Missing audio in Update 13

I (and others) have been experiencing the problems below on PC (NA) since Update 13. Kinda surprised it hasn't been addressed yet, since they do stand out quite a bit, so I'm going to try reporting them here:
  1. Some audio clips are not playing. Instead, we hear a synthesized voice describing the audio that was supposed to play. For example:
    • After the puzzle/cage room in Ruins of Mazzatun, instead of hearing the sound of the exit door unlocking, we instead hear something that sounds like "<Unlocking Sound>", which the game renders literally as "Less Than Unlocking Sound Greater Than".
    • Similarly, after the chain-pulls in Maw of Lorkhaj, Adara'hai's song is missing and replaced by a synthesized description.
    • After killing Rakkhat in Maw of Lorkhaj, we hear something along the lines of "<Final Verse>" (or "Less Than Final Verse Greater Than").
  2. Background music is missing in some instances. For example, Dragonstar Arena--we typically hear the dolmen music while in DSA, but now we just hear... nothing. There are other instances too with missing music; DSA was just the first one I could think of off the top of my head.
