Not all bugs can be found on the PTS since the coding is different. When they do a potential patch on the PTS the do a clean install of the game. SO it doesn't have millions of extra data in it from players or other various things. So bugs can slip through and this is just one of the reasons that it could have.

Correct me if I'm wrong, but this patch was never pushed to the PTS. In fact the vast majority of these incremental patches never go to the PTS, they are 'tested' internally, then pushed directly to Live. It's understandable for bugs to slip through here and there, and with small bugs it's generally of little immediate consequence. This, however, is no little bug ... they have essentially broken the game with an 'incremental' patch then never saw the light of day on PTS ... perhaps they need to either start testing these incremental patches on PTS to get a better idea of issues before it goes to Live, or re-evaluate the way they do internal testing. A bug that breaks a large portion of customers games should never get released to the Live environment.
