I suspect what’s happening is that the vampirism overlay takes up the “skin” slot (hence why applying a skin hides vampirism). The skin slot is applied over the body markings slot. What would be nice is if they were to add a “body texture” slot that goes under the “skin” slot where vampirism, beastfolk scale/fur patterns, and the elf/man skin textures can go. The downsides would be that people who like paler body markings or use a skin to hide vampirism would have their characters changed. But we also would benefit from a “hide vampirism” option anyway (it’s not like it is necessary to see vampirism in, say, pvp… you can just wear full armor and no one would know), so then it’s down to liking the pale body markings (which could be replicated by making a pale skin. Like the scalecaller peak skin, though that one also gives you hella skincare & necromantic body markings).
