Not sure on the first point since I've never paid attention (though I would assume whatever bar you have active upon transforming), but Weapons themselves don't matter after you transform since WW doesn't have access to Weapon Passives.

I also personally tend to set my stuff up so my Weapons are the same Set, which just helps me not have to keep track of which Set I'm on at any given time.

You keep the weapon bar that you transform with. If you transform on your front bar, you'll continue to get the set bonuses for things on your front bar.

Note that there is a bug with the Monomyth Reforged set which can sometimes give you the incorrect bonus.

Regarding your build issue, I would verify that you have 5 pieces of that set active on your front bar while in human form. If you transform on your front bar and lose access to the 5 piece set bonus, I would submit a bug report in-game because that's not supposed to happen. It might be a similar issue to the Monomyth Reforged bug where it thinks you're on the incorrect bar.


As far as the weapon itself goes, you get the trait, enchantment/poison, item set bonus, and base damage/armor (if shield) from it when in Werewolf form. The only thing you do not get and are not supposed to get are any Weapon passives, which are intentionally disabled despite you meeting the proc condition (having the weapon equipped on your active bar).
