Yeah, you have to pick one, if one is available in that value range - if not it will just go to the played stack without effect. But that card you are showing off is an agent, if you don't activate it, it will do nothing, as well not provide that gold coin.

But why would you not want those 2 cards highlighted - that is exactly that type of card I often go for

There are few reasons to not want a free card, of the top of my head:
- Messing up your deck / potential combo breaker. As Hlaalu I often bank on double patron combo, wouldn't like that messed up.
- You can destroy it later of course, but you might want/need to use other patron instead.
- These cards are not desirable for opponent either, but taking them might reveal something good for opponent, that you can't get rid of right now

But left one is decent indeed even as "solo" psijic card
