Planesbreaker Still Broken

Hey quick update on Planesbreaker and its current condition. My group and I have been progressing this trial since Day 1 PTS. Since then we have raided 3 times a week up until now to which we are on Day 98.
On the 23rd of January we achieved a 36/36 with 0 deaths with all Hardmodes enabled with a trial time of 26:31 and a score of 291823 however when in the achievement tab we were not given the No death achievement which meant we could not receive the Trifecta Achievement.
There is a bug that can occur when an enemy Militant add would get itself stuck in the wall near the mini boss Basks-In-Snakes making it unkillable
However we made sure to send someone over each run to make sure it came out and wasnt stuck (That person was myself) If it was stuck it meant we had to reset the instance.
Below are 2 examples from previous runs of the Enemy Militant getting stuck in the wall stopping us from killing it.

However in the video linked below at 1:00 you can see the Militant coming out of the area it gets stuck in along with the Tank in the group taunting it.
