For me when tanking pugs it is when players think because they are getting attacked by the boss because I am not holding aggro. For example, today in my veteran Spindlecluth I pug, I had a player become very toxic towards me with messages because they were getting randomly attacked by the spider boss. I tried to explain that I am using the addon untaunted to keep 100% aggro but for the spider boss mechanics the boss does random attacks to the group players even though the tank has taunted the boss and currently holds aggro lol.

Really hard to narrow it down. From casting any spells/ults right on top of the boss so I can't see him. Or pets that have some mob on the other side of the room so I can't chain it in. Or the dps that runs into the room AOE'ing everything which only scatters and gives cc immunity to the ranged mobs. Monster helms that spawn pets right in front of me. Anyone that runs far out of range anytime a boss even farts in their direction. People that stand at max range that will cause the boss to leap on them so I have to continually reposition the fight. People that only watch their rotation and never what's going on, like rezzing, standing in red or not interrupting that one shot pin. That's just a few off the top of my head.

Those big bright spells tend to have an AOE affect. Try casting the center of it behind and to the left or right of the tank so the boss is still in the ring of it, but the flash is behind the tank.

Maw of the Infernal / dropping ult in front of the boss. Drop that crap on the bosses butt, tanks can still hit any synergy just fine.
