Only heavy sacks in the Imperial City have a tiny chance of dropping one or more of them. (Happened only once in all my years there)
Molag Bal also has a chance to drop one. Trove Scamps in the sewers can as well.

Other than that, only via purchase or psijic portals in the wild.

I'm in the upper city almost every day. I've never seen hakeijo drop from a mob or a boss in the upper city, or a mob or boss in the sewers other than the Trove scamps.

I don't doubt this, I just don't fight the simulacrum often enough to have seen this myself which is why I didn't have it in my list.

When I go to IC I only do the dailies, I've been saving my tel var for war torte recipe so the only hakeijo I have is dropped from mobs. The drop rate is low, for sure, but its there.
