First boss in Vaults of Madness, where it seems players always end up killing one of their own hapless party members when it does that thing where it redirects all damage to one player.

What? The first boss is that little Head Shepherd Neloren who used to be tough a long time ago when she had two buddies to heal her, but now she is a walk in the park.

NGL I've been playing for over two years and I STILL have no idea how the Planar Inhibitor boss in WGT works...
Harrowstorms: A lot of people don't seem to know that you don't have to kite trash mobs around the perimeter while you wait for people who know what they're doing to show up. You can actually drop DOTs on the Witch Pikes and then kill the Lost Souls to make actual progress toward beating the thing.
Don't know how many of those I've rode up to where there are 10+ people running around in circles while all three pikes are still up with 100% health.

In my experience, almost everyone that can complete vet Tempest Island does that mechanic properly.

I had to solo this boss once because no one in group was able to understand it
