Why aren't enemy players "flagged" as such anymore?

Players of enemy factions used to have a small icon over their heads with the faction symbol and red outline. Well no more, they look like friendlies unless I mouse over and then there might be a tiny red outline if I look well (and the room I'm in isn't too lit up). Why did this get changed and how do I change it back? I have no addons that affect my UI so that's not it. I have done nothing to change my settings in any way since I last went to IC.

For me, enemy players still have the flag of their alliance over the head. Maybe its a setting of yours idk.

Pretty sure you can change whether alliance indicators show in the game settings, not sure which tab though.

This. There are settings specific to enemy players for showing health bars and the alliance icon. And I do not recall which tab either.

Except I haven't touched anything in my settings for months, like I added the latency thing in March and that was it, and they were there when I was in IC just before Greymoor launch. Guess I'll mess around later and see if I can find out what got changed, I just do not understand why it changed on its own.

Probably changed since the any faction any alliance come in.

Settings change on their own. Good luck getting it straightened out.

Why am I not surprised that this is a thing that happens. Guess I better screenshot my defaults then.

I'm pretty sure settings are per character as well (at least in my observation on Xbox), so changing characters can also be the cause if you haven't set them up the same. Its slightly annoying as I always forget what settings I need whenever I make an alt. But if its not like that on PC or if its on the same character then I don't know.

I think you'll find the setting you want under Settings > Nameplates towards the bottom of the section.

But that's it. I have all my characters exactly the same. I even try to make skill bars as similar as I can across characters, like spamable always to 1, (self) heal goes on 4, shields on 5, ect, to make my own life easier. Settings looks, or should look, the same across the board. Haven't checked anyone else this morning, didn't have tje time, but my stamcro I did ICwith last night was fine, then I rolled in my magsorc there to get the final 6 dailies for the event and... this! Man I do not get it. But I guess that's ZOS for you.

Every now and again, I'll have my settings completely go wacky: enemy alliance indicators turn off, quest giver tags turn off, target glow turns off, all nameplates disappear. I can't seem to figure out why exactly. It might be related to Screenshot Helper but I'm not positive.

I know Screenshot Helper does bug out every so often and forget to turn some things back on. Usually if I notice there are no quest markers anymore, it means that all of those are still off and I'm stuck in the 'screenshot everything is off' mode. I just go and reset the defaults on the combat and nameplates tabs and everything's fixed back up.
