I understand this exact same issue happened when they released Elsweyr last year. Who is in charge of testing these things before they are released?

Think it was I who pointed it out, had been smarter too give all players on PTS the chapter the same way they give all ESO+Greymoor was unlocked for every player on the PTS, so we did test it but someone forgot to remove those permissions before it went Live? Someone in another thread mentioned when this happened with Northern Elsweyr Chapter (lol yes they did it again) they had to manually remove all players who shouldn't be in the zone from it and that is why it takes so long to "fix".
With something this big I can totally understand over-looking the "did you check the permissions settings" (if this is what caused it of course). Yes, they did the same thing last year with Elsweyr BUT they've also had to adapt to the pandemic restrictions and we're all human, between adjusting to work from home and finish this chapter the note from last year about chapter permissions got lost?
I hope none of the folks working hard to fix this issue are reading the forums though (after this is fixed of course) as there is SO much negativity! Yeah, we have to wait. Yes, it is annoying because we are all super excited to get to play Greymoor/Update 26 but some people are getting a bit extreme. I am sure after this is solved there will be an internal review as to how the same issue from last year happened again this year (assuming the same thing caused it).
I wish the best to everyone involved in fixing the issue. Don't worry about the negativity here because 5 minutes after the servers are up everyone will forget they had to wait and find something else to complain about
