That's extremely common in PvP when combined with cast time abilities + sneak/cloak. Fighting someone while cancelling animations extremely fast can also do it.

Yeah, it’s a semi-regular PVP occurrence. When it happens every few days I generally make a joke about turning on hard-mode.
Honestly, even though it’s happened to me probably 50 times over the years I’m still not 100% sure if it’s actually lowering your health or just a visual error. I think it’s the later.0
