Is just me or i cant find more quest In Shadowfen for the achievement?

well i complete every single quest in shadowfen and the achievemente still says 58/62.. anyone have the same problem???

I'm sitting at 57/62, so I clearly missed one, no idea where where it could be, or the other 4 for that matter. Everything filled in on my map, and i've searched everywhere.
Also missing 5 quests for the rift quest line, and haven't filled in one area where there are no quests whatsoever. Then again a quest is bugged, so that might have to do with it.

MrNiceSoul ShrivenIn Shadowfen I have 59/62. There is a quest which you can take jn the road which goes down (down-middle part of the map). After doing a quest you must go at the same place, where you took quest, but not go to the arrow (it's noone there).
In Rift I have bugged quest at the left top corner of the map, but my guildmate completed it and he also completed quest in broken fortress down of the map (don't know how to say). Didn't ask him about how many quest he has.0
