I think some sets have exceptions, and I don't know if Ashen Grip is one of them, but these are the general rules:
Direct excludes damage-over-time abilities. Though if an ability's description has initial damage that's separate from its damage over time, that initial damage counts as direct. Anything affected by master-at-arms according to this spreadsheet is direct.
Enchants procced by direct damage abilities count as additional direct damage, though I don't know whether enchant procs also count as melee.
Melee excludes targeted ranged and ground abilities, even if the target is nearby. Templar jabs are also excluded (at least for the Twin Blade and Blunt axe bleed passive) because they reach too far. I've seen the cutoff suggested as 7 meters (jabs reach 8), though I've also seen claims that Steel Tornado is melee regardless (9 meters), but I haven't tested that.

The description say:
“When you deal direct melee damage, you have a 10% chance to breath fire to all enemies in front of you for 1544 Flame Damage. This effect can occur once every 4 seconds”
I’m thinking it’s going to go off when I use a light or heavy attack (I’m using a great sword) and only a low level toon so using light and heavy attacks a lot and so far I have only seen this go off once
At level 4 Trainee is by far the best set. Unless of course you don't want to level up ...

That’s terribly incorrect.
Melee is any single target attack with range <=10m. Flame Whip, Burning Embers, Concealed Weapon are all class magic attacks that are melee attacks in game mechanic. If the skill is a DoT, only the first attack that procs it counts as direct melee damage.

My toon is on level 4 and I crafted level 4 gear, because why not. I know it’s not the best but just wanted to try it out for something different

Was referring more to twin slashes, whirlwind, flurry, etc. in the Dual Wield 2H skill lines, as I said. Most of those fall under physical damage, which is melee in terms of proc sets. Concealed weapon is a nightblade skill, Flame lash and Burning embers are a DK skills. They do not fall into the physical/melee category. We are talking about 2 different things.

You said melee attacks = weapons strikes. That's just wrong.
I don't know why you're suddenly trying to frame this discussion on a physical damage context. The OP asked about the Ashen Grip set's proc condition which is melee attacks. Full stop. That's regardless of type of damage dealt.
