They equal the same as killing blow medals.
I've had the most success getting them when I have Major and Minor Mending both applied and I let my teammates health drop a bit.

You need to play a better healer. Sorry but in my honest to god opinion 0 assits & 32k damage is inexcusable, esp for TDM. The recent change to healer medals has been a god send and most of us frequently land 1st or 2nd. It is most definitely not 100 percent dps focused. You meed meaningfull burst heals which account for alot. The scoring system attempts to not reward unimpactfull healing such is long running minor hots etc etc.
You need to use clutch burst heals and you need to have some form of actual lethal.
