When questing with my group/friend, we cannot see each other when enter some area's. For instance when we go to the cheesemonger. once we enter the instance, we do not see each other and must quest alone.
Can you see the arrow above the other person's head but they are invisible? If so, use the "Teleport to player" option on the Group menu to fix that.
If you don't see the other person's arrow, then it's a phasing issue (i.e. that the game thinks you are in different parts of the quest, so won't put you in together).

Some places, like Cheesmonger, are solo instances, and you can't take friends. I don't think this is intended behavior, since I've gotten into some of the solo instances with my girlfriend.

That's correct.
Some locations can only be completed solo. This is usually the case for important story or guild quests, although some regular quests are the same way.
