In the vein that people somehow think jumping should cost stamina, we can go the realism route here too.
Characters will have the “major need” debuff applied if they didn’t visit a toilet in the last 4 hours, reducing damage, healing and blocking by 50%.
Crown Store exclusive toilet furnishings for your own home, extending the time to 8 hours. Crown crates have radiant apex toilets, extending the time to 24 hours.
On the world toilet day, all toilet furnishings are 30% cheaper. Wouldn’t that be awesome.

As I've never seen anyone urinating in ESO, unlike in Age of Conan, or defecating, I assume they have no need for such things. I dread to think of the additional possibilities for role-players if there were toilets, however.
