Bug Report: Lost Equipped Helm while Crafting

After a second occurrence, I noticed that while crafting(blacksmith) in my case, my equipped helm disappeared. Whether it was through Researching/deconstructing/ or refining ore, my helm was lost.
Please be careful when crafting, equipped items could be lost spontaneously.
