Those vampires are the exception, not the rule. 99% of vampires aren’t able to control themselves like that. Kind of goes against the entire concept. Even morally “good” vampires like Gwendis struggle to keep their appetites in check, and that’s while being reasonably well-fed. Not really tradition in TES. There's a few vampires who have explicitly gone without feeding for extended periods of time, and are still quite sane. My impression was that bloodfiends simply went insane from their hunger, not that they starved themselves to that point.
