This is an new dungeon finder bug introduced then they added level restrictions for dlc dungeons.

Kind of throws a wrench in the grouping activity if you can't participate with a group of friends/guild.

This one is caused by an offline player that gets kicked. You can easily reproduce this bug. You are actually waiting for the server to kick the offline player out. Think ghost in the server.
FYI to all most of these problems happened when Homestead Patch came out.
