  There's a thing with some addons where they show custom titles for some users.
  
  I think it's the Skyshard/Lorebook ones that do it.
  
  Basically the person asks the devs of those addons politely for a title, they then enter that name into a database along with a default title, say, volunteer, and then anyone with the addon will see that person with a custom title instead of the default one.

  What he is trying to say is that there are numerous addons that the developers code "hidden titles" into the UI. The most popular one is lui extended. So if you are running one of those addons that has some stuff coded in, certain players can have custom titles that your UI will show them as having. Unless of course you are not running any addons at all, then a custom title is not possible.
