In the Belkarth inn, the guard walks upstairs while patrolling, stands next to a pillar, then walks back down. I've been caught a few times.
That said, Belkarth is excellent for thievery - low npc density, chances for line of sight lifts, and a quick escape route down the hill. I do a round when I'm in town for writs.
Step 4.) Use your Blade of Woe and murder the two NPCs that you find there. It's preemptive, because snitches get stitches. If you don't have the Blade of Woe, then head to Gold Coast and acquire this ASAP.
