SolarCat02✭✭✭✭✭I have run into this bug a couple times, most memorably with a greatsword and a pair of pants. I don't remember exactly where I got them, but that's in part because they were just a mob drop.
The pants were in a delve somewhere in the Pact I believe, off some random archer. Maybe he was a skeleton. The greatsword was somewhere in Covenant territory. I don't remember where.
They gave very low stats (looked like maybe level 1 values), were invisible when equipped, and gave nothing whatsoever when deconstructed. They had the red question mark icon as shown in your screenshot.
I just figured that every once in awhile the RNG would get confused and forget to set a value for something.Why be normal when you can be better?

You cannot deny that this game is beautifully rendered and minor details like the thumbnails on the tool-tips are always increasing in quality. I have nothing but respect for the artists and other graphics renderers of the game as they do quality work.
