Coming from a person who has no knowledge of coding - I'm wondering if adding a difficulty option for the non instanced portion of an MMO, is even easy to do?

I've played many MMOs and I don't ever recall having that kind of option available to me while I'm free roaming (non instanced).

    Typically, if you're a level 15 in a level 45 zone(for example), you're going to get clobbered.
But since scaling is ESO's "thing to do" , it's now normal to be at whatever level and face whatever enemy out there, regardless of which zone you're in. Not all zones, but soon to be all with Tamriel One.

I assume adding a difficulty meter option for each player, would be quite complicated to do, no?

Or is it as simple as what Diablo 3 does?
    Example-
Boss A hits for 5k damage on easy
Boss A hits for 20k damage on hard
Boss A hits for 50k on Torment 1

Reducing a players stats by a percentage with each difficulty tier may work.

    Normal - unchanged player stats

    Hard - -15% health +15% damage taken, -15% Spel/weapon Damage, -15% recoveries -15% resistance

    Nightmare - -35% health +35% damage taken, -35% Spell/Weapon Damage - 35% recoveries, -35% resistance -50% healing taken

    or something like that.

Go back to the start. Why not role a new incompatibly built character, like an Orc NB healer or a Wood Elf NB tank, do not use any CP and use only gear, food and potions you can find (not buying anything with your account gold).

Instead of wearing no armour, you can be a stamina build using all light armour or even see how far you can get in your prison rags you wear when you start the game. Do not slot any skills on your bar and simply use your weapon with light and heavy attacks only.

I am sure there are countless ways to make the game challenging. Remember that as soon as you start getting sick of dying, that was your intention all along - to make the game tough.

Overall survival-hardcore system(note that this is currently possible through roleplaying)
    - Have weather influence your character(buff and debuff, no need to radically criple the questing)
    - Basic needs(sleep deprivation, hunger and thirsty debuffs)
    - unknown diseases caught on the wilderness and that need to be treated only by town and/or specialized healers
    - Limited backpack space-based inventory and incumbrance system, including only two sets of items/clothing and limited amount of ingredients, potions, and junk you can carry at a time
    - Make some quests and world events have requirements based on real Tamriel calendar(like summon the daedra in early TES)

*Reputation system, the characters actions opens and closes doors(questlines and skill-lines)
*Real regret(Oblivion Tribute) - travel Tamriel in seek of forgiveness for your crimes - pray at temples and shrines to change alignment/increase reputation

Mandatory system changes(not possible through RP)
    - Be able to yield a torch(and having dungeons require that you to)
    - City routine, with NPCs closing and locking their business and homes.
    - Limit horses to wilderness or only outside the cities
    - No trivial resurrection: shrine-respawn only system and very high demanding costs for bringing someone back

    Revamped Justice System:
    - Guards restrict suspicious behavior, and if one sees you sneaking around town he will put a light bounty on you and will demand you to pay a fine and stop such acts;
    - Once you start a fight in town, not only guards and the attacked NPC will become hostile towards you, but some more brave NPCs, Fighters guild agents and other members of neutral-good factions
    - Hunters and assassins for characters with a high bounty or guild rank
    - jail system(TES tribute) for the criminals caught by the guards, with prisoner arenas for public amusement

    No.

If you want a challenge then challenge yourself, strip naked and use iron weapons. If you want extra challenge then strip your champ points.

I like the moderate challenge that the normal world offers. For someone who doesn't use the best sets, doesn't have gold gear and isn't anywhere near the CP cap, the game feels right...not too easy and not rage inducing, dying to proper bosses solo and destroying normal mobs.

This is what spoils the game , there is no moderate challenge no incentive or need to group, most of the so called vet dungeons are simple 15 minute face roll without the need for proper set groups.and the same goes for most of the so called world bosses except for a few..
