i don't have lag often, but when i do, its happening to other adjacent players of the same team.
Playing from singapore and usually have consistent 250 ping with 9999 spikes occuring in certain times during PVP. There's some association with lightning spells.

What does affect ping is distance, path taken, line connection type (Copper, fibre, coax, wireless), and all the equipment and line types from your premises to the destination.
As for ESO, I consistently run 10ish add-ons on ultra settings getting 50+ FPS and only encounter lag when there's large fights going on, which is server side lag.
1.5 Mbps here. I lag all the time. Mostly due to the fact it's a shared connection. I live way out in the sticks (USA), so I only have access to one lousy provider (CenturyLink). It really sucks though because less than two miles down the road a friend has access to a 10 Mbps connection.

I recently did some tests in Cyrodiil with some network traffic software. I ran to find the largest zerg I could and participated in skill spamming for a solid 15 minutes.
What I found was that total network traffic received never exceeded around 60KB/s (which would be around one third of your 1.5mb/s speed), but the server was still lagging out at a 400-800 ping rate (usually around 80-100 for me).
What this means is that even with your lower internet speed, you should still be able to play the game just fine. It is not your connection speed. It is the servers not giving you information fast enough.
