Ok. I was under the impression there was a cap on how many players you can have blocked

Do the blocked morons stay blocked?
Are they unblocked if you logout or change characters?
Is there a way to unblock once you block someone by mistake?
Are those you block also blocked when using your other characters?
I am on PS4. I assume the mechanics are the same on XB1.

You can start by lowering the volume on voice chat as the default is to loud. press options button > options > player voice, bring the dial down so players are the same volume as the npc's.
Loud or music players - first you must identify them, there are two ways of identifying players, 1- there names are on the screen with they are talking 2- they have a circle symbol over there heads, now that you got there name.
Go up to the player and hold option button, select view profile. there PSN profile will pop up, look for the symbol with the three dots and select block player.
If you cant find the player press option button > social > voice chat > with area highlighted press triangle, look for the players name and press X then scroll down to Block.
Be sure to do this with every bad seed you come across and sooner then later your voice chat will be under control.
This is what Ive been doing and its made towns 100% more enjoyable experience for me.
If you keep up with this you will see that the loud or music players are few and far between and that the majority of players are polite and helpful.

Oh yea I expect people to tell me if my mic is too loud or crap in guild, just as I tell others.
The worst is having something on the comical side erupt in guild chat and old mate with the full on headset blows your ears out.
