Yes, you have to separately buy a DLC for any account you wish to use it on.

Also, you don't have to continuously pay a subscription fee on any account in order to play the game at all.

Not only account based but also server based. In same account you have to purchase 2 times if you want dlc in both servers.

Of course if you're an ESO+ subscriber you have access to it on both servers automatically without spending any crowns... But only for as long as you stay a subscriber.

Since PC and Consoles are separate now, you would have to get one on both to be able to play it on both. That would be the same for any two accounts regardless if they are on the same platform or not.
