Do you happen to be playing a Templar?

I use it on my V14 Sorcerer as well works like intended. On my V12 Templar that is another story. Most of the time I can hit the "R" key till death and never get the bats. Rarely does it work when I press it the first time. This is in combat only. I can test it outside of combat no problem fires first time every time. It does seem intermittent but it is repeatable.

ZOS can you please confirm that this is a known issue?

I disabled all my addons (wayshrines, AIRG) and reloaded the UI. Did 5 tests, Used explosive charge to the mobs then hit devouring swarm;
First test was Scorpions and it worked first press, Next test scorpions failed pressed till dead no bats, Next 3 wasps failed pressed till dead, next 8 wasps fail press till dead, last 5 scorpions fail press till dead. These were all done in upper Crag. 1 out of 5 worked as intended.

This is going to seem like a silly question, but, for science.

Have you removed the ult from your bar and re-slotted it?

Also, have you placed the ult on your second skill bar to see if it works from there?

Both silly questions, but may lead us to some answers on what's happening.

Just for Science I did as you asked and took a few more deaths. It did not matter what bar was used and I did reslot the first bar. What was interesting was that I observed 2 things:

1. While sitting there dyeing and smacking the ult key the icon in the hot bar actually looks like it is being hit over and over but no bats.
2. It seems that it has worked on the first test every time. This would be right after I log on but not the subsequent. It would take a few more log ons to see if this is in fact true but it appears so.

Let me know anything else you want me to try, I would really like to get this working. The Templar has had a rough roe to hoe so far.

No I am not a Templar. I am a Sorc and this was messed up for me a few weeks ago and I started a thread when I noticed it. Nobody from ZOS responded so I have cured Vamp. That said I am unable to further test it.
For the record it did not matter what bar it was on, whether I removed it or not. I could use it out of combat just fine, but when I am fighting for some reason it just wouldn't go off.

@ZOS_GaryA‌ well I have a new one today, I played the Templar for just a bit to see if anything has changed. It did, I couldn't fire the ultimate at all. Standing still not being attacked it would not fire. I'm talking like at a wayshrine, nada. I relogged, same thing. I removed it from both bars and reloaded UI, same thing no Ultimate. Was really weird, It made the sound of it starting and my character looked like it was going to start it, the button looked like it was pressed numerous times. But alas no bats.

Was going to try a different Ulti so I slotted Nova and headed to craglorn to beat on something to get it up. And wouldn't you know it bats fired.

There is something really screwing going on in the Vamp/Templar lines. Please let meknow what I can do to help you fix this junk. Other wise I have a V12 that is Junk.
