Game crashes when I press the 'Play' button (fresh install)

I recently formatted my main computer and now when I try to play ESO it crashes with a black screen right after pressing the 'Play' button from the launcher. The game was working fine before my format. I'm running Windows 7 64bit with a Radeon HD 5850 video card. From my searching, I have tried the following:

Repair install
Complete re-install
Marked both the Launcher and ESO executable to run as Administrator
Marked both the Launcher and ESO to run as XP Sp3 (since unchecked)
Added and changed the usersettings.txt file (since deleted)
Updated video drivers to current beta release (same as before my format)

I recall during the transition from Beta to Head Start I had this issue (or a similar one), but I can't recall what I did to fix it. The game has never made it past the opening cinematic since the re-install, so I believe that is what is crashing it. If anyone can offer any assistance, I would greatly appreciate it!

    Hey there @brent_doughtyb14_ESO,

    Big important question for this one. A few, actually.

    1. Do you have multiple hard drives?
    2. Which drive is ESO installed on? The default C: or the alternate drive?
    3. If you have multiple hard drives, which one was wiped?
